INDIVIDUAL PLAYER OR TEAM REGISTRATION
WAIVER & RELEASE OF LIABILITY
1. Assumption of Risk. I acknowledge that basketball involves inherent risks, including but not limited to collisions with other players, falls, slips, court surface hazards, and equipment-related injuries. I voluntarily accept these risks on behalf of myself. I am solely responsible for my own (or my child's) medical costs resulting from participation.
2. Release of Liability. I hereby release, waive, and discharge Bellas Basketball Alliance, coaches, volunteers, employees and the venue owner from any and all claims, demands, or causes of action arising from personal injury, death, or property damage, whether or not caused by the negligence of the foregoing parties, to the fullest extent permitted by law.
3. Indemnification / Hold Harmless. I agree to indemnify and hold harmless Bellas Basketball Alliance and its representatives from any legal costs, fees, or expenses arising from any claim I may file in connection with participation.
4. Media Release. I grant Bellas Basketball Alliance the right to record, photograph, and broadcast images and video of the participant during league games, open gyms, and events. I authorize Bellas Basketball Alliance to use, publish, edit, and distribute these images and videos on social media, its website, promotional materials, and other media without compensation or further consent. I may revoke this permission at any time by notifying Bellas Basketball Alliance in writing, in which case I request removal from future posts.
